Lens replacement surgery, or refractive lens exchange (RLE), is a procedure that replaces the eye’s natural lens with an artificial intraocular lens (IOL) to correct vision issues like presbyopia and high refractive errors. Suitability for this surgery depends on factors such as age, vision stability, eye health, and general health. It’s most commonly recommended for individuals over 40 with significant refractive errors, but may not be ideal for those with advanced eye conditions or unrealistic expectations about the outcome.

This surgery offers long-term vision correction, prevents cataract development, and provides fast recovery. However, a comprehensive consultation with a qualified eye surgeon is necessary to assess individual suitability, discuss potential risks, and set realistic expectations. How Does Refractive Lens Exchange Surgery Work?

Preparation

Before the procedure, the eye is numbed with anaesthetic drops. A sub-Tenon’s block or general anaesthetic may be used for added comfort or in cases of patient anxiety.

Lens Removal

The natural lens is gently removed through a small incision.

Lens Replacement

An artificial IOL is inserted to correct vision.

Recovery

Most patients experience significantly improved vision within a few days.

The initial pre-operative consultation is the most important stage in your journey of spectacle independence.

You will meet a specialist optometrist who will discuss your medical history and current lifestyle along with your initial interest in vision correction. You will be exposed to a number of diagnostic tests by your patient advisor, using cutting-edge technology to measure vital results which will be assessed by the specialist optometrist to confirm your initial suitability to a procedure, pending ophthalmologist (eye surgeon) final decision.

It is strongly recommended that you bring a friend or family member to this appointment and all subsequent as your eyes will be dilated resulting in your inability to drive for 4-6 hours. In our Manchester and Birmingham hospitals we do have a coffee shop where you could wait for the effects of the drops to wear off, before driving, if needed.

Should you be a successful and suitable candidate for the procedure you will have the opportunity to book your surgery at this appointment. Please note that you will be required to speak to your surgeon either in person or virtually at least 7 days prior to your surgery date. Most patients pay a £500 deposit at this appointment to secure a surgery date.

At this appointment you will meet, usually virtually, with your ophthalmologist. This is always at least 7 days before your surgery date, giving the ample, legal timeframe for you to be fully consented and have enough cooling off time to decide that you are completely happy to move forward. It is the consultant ophthalmologists’ job to make you aware of ‘both sides of the coin’. This being; the significant, life changing benefits of visual freedom, but also the potential side effects, complications and risks that all surgery carries. Our ophthalmologist will go into more granular detail about your chosen surgery and its pros and cons. We find that our patients appreciate the ethical approach that Transform Vision take to the consenting process, where our ophthalmologist is given complete agency to decide who is (and who isn’t) a great candidate for treatment. If your surgeon advises you against treatment, any monies paid will be refunded. Most patients are excited to know they have been given the final ‘all-clear’ to move forward by the surgeon at this appointment, and are a little nervous, but excited to move forward with surgery.

Just before the operation you will be given some dilation drops to enlarge the pupil. This will shortly be followed by an anaesthetic to numb the eye in the form of eye drops. A clip holding your eyelids apart allows you to blink safely during surgery.

Your surgeon will be looking through a microscope to perform the surgery. A small self-sealing entry point will be made at the junction of the white of the eye and cornea, the lens capsule will be removed followed by the removal of the natural lens via a high frequency vibrating probe, fluid washing and vacuum. The new artificial lens will be placed into position finished by a wash-out and refilling with fluid and antibiotics. You will be awake during the procedure but you will not be able to see what is happening apart from a bright light. During surgery you will be asked to lie as flat as possible with your head still. The operation usually lasts 7 minutes but can last longer. A pad or shield may be put over your eye to protect it.

This is a local anaesthetic procedure, usually performed to both eyes on the same day. You can go home shortly after your surgery is finished, after you have had time to rest and been given your TTA (to take away) medications. Getting a lift home or a taxi / planned transport is best. Most patients can see to drive after 1- 7 days after the operation. Your optometrist will call you the next day to do your 1 day check up, and ask you to come in to the clinic if we need to see you face to face based on how you feel you are recovering. We then see most patients after 1 week in clinic and we can show you the visual improvement on the wall and reading charts. Your aftercare will follow on from this point forward. Most patients are discharged from our care within 6 months.
